Our clients can fill out these forms with information on the doctor requests and send the forms to Shu Dental Lab with the patient's case. In addition, there are downloadable diagrams of our products and services.

All cases picked up from your office we receive the following day.
If a return date is not specified on your prescriptions, we will automatically process the case for nine business days;
this does not include weekends or holidays. Calendars are just estimates for delivery. Thank you for your cooperation.
